Annual Update of Information Forms:

If you have a student returning for the 2020-2021 school year, then please see instructions below

Fill out Annual Update of Information packet (PDF Packet)


  1. You can pick up a packet at the school site
  2. Email to have the packet mailed to you
  3. Make sure you sign and date all pages
  4. If your physical address has changed, make sure you include Physical address verification (utility bill or equivalent)
  5. Methods to return completed packet:
  6. a) Scan and email to
  7. b) Deliver to the school site the student is enrolled with
  8. c) Mail to the following address and it will be forwarded to the intended school site:
